Sports activities are integral to a healthy lifestyle, but they come with inherent risks, including injuries that can lead to surgical procedures. A commonly seen sports injury involves a tear in the anterior cruciate ligament (ACL), often necessitating an arthroscopic ACL reconstruction. As an experienced orthopedic surgeon, I feel compelled to shed light on the various sports injuries that can lead to ACL tears and the reconstruction procedure.

Understanding the ACL

The ACL, one of the four major ligaments in the knee, is instrumental in providing stability and restricting the tibia's forward movement in relation to the femur. An ACL tear can substantially disrupt knee stability, causing difficulties in performing movements often found in many sports.

Common Sports Injuries Resulting in ACL Tears

A variety of sports injuries can lead to ACL tears, with the risk significantly heightened in certain sports:

  • Non-Contact Injuries Most ACL injuries surprisingly occur in non-contact scenarios, such as running, jumping, or quickly changing direction. This sudden shift can lead the knee to hyperextend or rotate inward, placing excessive strain on the ACL, resulting in a tear. Sports like basketball, soccer, skiing, tennis, and gymnastics commonly witness non-contact ACL injuries.

  • Direct Contact or Collision Injuries ACL tears can also result from direct contact injuries. These typically happen in sports involving substantial physical contact, such as football, rugby, and martial arts. A hard hit to the knee's side can result in an ACL tear.

  • Incorrect Landing from a Jump ACL injuries can also occur from an improper landing after a jump. If the knee is straight or slightly bent upon landing, the impact force can be potent enough to tear the ACL. This is often seen in sports like basketball, volleyball, and skiing.

Gender and ACL Injuries

It's important to note that females are at a higher risk of suffering an ACL injury compared to males. This discrepancy may stem from differences in physical conditioning, muscular strength, and neuromuscular control. The wider female pelvis leading to a greater knee joint angle (Q-angle) might also heighten the risk of ACL injury.

Diagnosis and Arthroscopic ACL Reconstruction

Typically, diagnosing an ACL injury involves a physical examination to check for knee swelling and tenderness. I perform specific tests to evaluate the knee's stability. Imaging tests like X-rays or MRIs may be used for a more detailed knee inspection and to confirm the diagnosis.

In cases of severe ACL tears or for those eager to return to sports activities, I often recommend arthroscopic ACL reconstruction. This surgical procedure involves using a graft, typically harvested from the patient's own body, to replace the torn ACL. Using arthroscopy, a minimally invasive surgical technique, allows for smaller incisions and specialized tools, reducing tissue damage, pain, and facilitating faster recovery compared to open surgery.
